The St. Albert and Sturgeon Primary Care Network Pharmacy Team is an integral part of a multidisciplinary team. The members of the pharmacy team are Melissa Dechaine, Tara Grimstead, Lisa Tate, Lindsay Torok-Both and Andrea Pickett. By working to their full scope of practice and embracing a team approach, this group of pharmacists has positively influenced the health and well-being of their patients.     

The team works directly with physicians, nurses, and other health care professionals within the Primary Care Network. This unique setting helps facilitate communication and collaboration across all health disciplines and ultimately enhances patient care. “We are more efficient and effective when we collaborate with both our pharmacist colleagues and the rest of the multidisciplinary team,” says Tara Grimstead. “We can do so much more for the patient when we work together.”

Just a small sample of what this team is responsible for includes a pharmacist managed anticoagulation program, a geriatric evaluation and management program, medication reviews, drug therapy monitoring, initial access prescribing, and patient education on various topics such as diabetes, asthma and heart health.

“As a team, we are always trying to improve our skills and knowledge to provide the best patient-centered care, says Melissa Dechaine. “This may mean optimizing drug therapy or could involve helping them to understand their medical condition, motivating them to achieve personal goals or even being considerate of financial or social implications when making recommendations.”
