The Institute for Safe Medication Practices Canada (ISMP Canada) has launched its updated MedError.ca website, aimed at helping Canadians better understand medication safety. Included on this revamped site is an updated online platform designed for patients and caregivers to easily and confidentially report medication errors. This information provides essential information for healthcare systems across Canada and contributes to systemic improvements in medication safety.
While ISMP Canada has long provided patients with the ability to confidentially report incidents, MedError.ca now also offers practical resources to help patients play an active role in their own medication safety. These resources include newsletters with timely tips, trusted tools like “5 Questions to Ask About Your Medications,” and patient stories that remind us how reporting incidents can create a ripple effect of safety improvements across Canada. By sharing these resources with their patients, pharmacy professionals can further empower and engage them in the safety of their care.
MedError.ca is part of the Canadian Medication Incident Reporting and Prevention System (CMIRPS), a collaborative, pan-Canadian program supported by Health Canada and involving other partners that include the Canadian Institute for Health Information, Patients for Patient Safety Canada, and Healthcare Excellence Canada.
