When a practice incident or close call occurs in a pharmacy, it’s an opportunity for the team to learn and make changes to help prevent it from happening again.
In our next CQI+ video, ACP deputy registrar Jeff Whissell explains how teams can analyze practice incidents and close calls to identify the contributing factors and root causes of the event.
Remember, pharmacy teams have until February 1, 2026, to meet all the requirements of CQI+. These include acquiring an online practice incident management platform and online safety self-assessment tool, developing policies and procedures to guide the pharmacy’s continuous quality improvement program, and holding regular continuous quality improvement meetings of the pharmacy team.
