The National Snapshot for 2024 from the Institute for Safe Medication Practices (ISMP) Canada is here, marking the most robust version of this publication to date. The National Snapshot shares an overview of key trends, insights, and information about the types of practice incidents and close calls reported to the National Incident Data Repository (NIDR) by community pharmacies across Canada. The NIDR was established to support the collection of reports of practice incidents and close calls submitted by community pharmacies for the purpose of improving medication safety.
As part of CQI+, ACP’s continuous quality improvement program, community pharmacy teams in Alberta will report all practice incidents and close calls to the NIDR. While many pharmacy teams have already started this, the reporting of practice incidents and close calls to the national database becomes mandatory as of February 1, 2026.
More detailed learnings from the reports of incidents are also shared in safety-focused publications, including ISMP Canada Safety Bulletins and NIDR Safety Briefs. In 2026, ISMP Canada publications, including the National Snapshot for 2025, will include data from the early adopters of CQI+ in Alberta!
