Prevent practice incidents involving prednisone and prednisolone

The Institute for Safe Medication Practices (ISMP) Canada has released its latest Safety Bulletin: A Multi-Incident Analysis of Reports Associated with Prednisone or Prednisolone Therapy. This bulletin highlights the findings from an analysis of medication incidents associated with prednisone or prednisolone, with a focus on community pharmacy, and shares opportunities to prevent and/or mitigate associated risks.

The bulletin shares key themes associated with reported practice incidents involving prednisone or prednisolone, key contributing factors to the incidents, and key strategies to prevent and/or mitigate practice incidents involving these drugs.

The information in the bulletin presents an opportunity for pharmacy teams to reflect on their own practices and consider what changes they could make to improve based on the experiences shared by others.

This Safety Bulletin is an example of how ISMP Canada uses information that is submitted to the National Incident Data Repository for Community Pharmacies (NIDR) by pharmacy teams, and how analyses can be shared and applied in practice. This type of analysis is only possible thanks to pharmacy teams contributing this information to the NIDR anonymously through a commitment to quality and safety. Submitting practice incident and close call details to the NIDR is a key component of CQI+, ACP’s new continuous quality improvement program, which is now required of all community pharmacy teams as of February 1, 2026.
