ACP provides a variety of practice tools for regulated members to use in their practice. The intent of these tools is to assist regulated members in meeting the standards of practice with respect to key patient care activities. Chat, Check, and Chart (CCC) provides guidance to pharmacists about engaging, assessing, and monitoring patients.
Included among these tools is the Vital to patient care resource which expands upon some of the core principles – such as using Netcare routinely as part of patient assessment activities – outlined within the Chat, Check, Chart process. Partners in patient care identifies the roles and responsibilities of pharmacy technicians in the Chat, Check and Chart process. Licensees should post these documents at their practice sites for quick reference, while also ensuring regulated members regularly review these principles to continue refining patient care skills.
