How are your patients affected by transitions in care?

October 21, 2014

The Safe Transitions in Care pre-conference will examine the impact of hand-overs to other providers and other transitions that affect patient care. Your input could help improve the healthcare system.

When: October 28

Where: Edmonton or online

This event takes just before the Canadian Patient Safety Institute’s National Forum on Patient Safety and Quality Improvement on October 29 and 30, and is co-hosted by Alberta Health Services, the Health Quality Council of Alberta, and the Canadian Patient Safety Institute.

The pre-conference day will feature local, national and international presenters on topics related to identifying and addressing gaps in transitional care. Transitions in care refer to points in the patient journey where an individual may see different providers, different programs, and where communication and care planning is of utmost importance.

Sessions and speakers are outlined in the pre-conference program.

Registration can be accessed at www.asklistentalk.ca. To facilitate access to the conference, both the pre-conference and main conference are also being broadcast virtually.
