A recent mix-up between methadone and buprenorphine-naloxone for a patient’s opioid agonist therapy led to an important safety bulletin from the Institute for Safe Medication Practices (ISMP) Canada. ISMP recommends pharmacies use standardized documentation templates to avoid such mistakes.
