Reporting of lost or stolen controlled substances

April 4, 2018
Health Canada’s new guidance document walks you though the steps of what you need to do if controlled substances are lost or stolen.

The Office of Controlled Substances has released a new guidance document: Reporting of Loss or Theft of Controlled Substances and Precursors.

The information found in the guidance document is effective immediately. The document provides instructions on the scope of what should be reported to Health Canada when a loss or theft is detected, and includes a fillable PDF form to facilitate completion of the document. The goal is to make the process of submitting this information to Health Canada easier, while also increasing the quality of the information by standardizing the way information is entered into the form.

A link to the guidance document can also be found in the Practice Materials & Tools section of the ACP website.
