TPP Alberta transitions to two-part prescription forms

March 20, 2019
New forms are now in use.

After more than 30 years of using a three-part prescription form to monitor drugs prone to misuse, TPP Alberta has moved to a two-part form to improve processes and save significant operational costs. TPP Alberta has asked prescribers to use up their existing stock of three-part forms before re-ordering. Prescribers using the new two-part form will give patients only one copy (the top copy) of the TPP Alberta prescription to take to their pharmacy. Prescribers will remove the second copy (bottom copy) from the pad and attach it to the patient file as part of the patient record.

The submission requirements for dispensers remain unchanged. Pharmacies will continue to send a copy of the TPP Alberta prescription only for compounded medications, veterinary use, office use, or when it is a Yukon prescription. In these circumstances, where a copy is required to be sent to TPP Alberta, the dispenser will need to make a copy to send in.

For more information on how to use the two-part form, see the program guide or easy, one-page reference. Questions? Email TPPinfo@cpsa.ab.ca or call 1-800-561-3899 ext. 4939.
