AHS seasonal influenza immunization program participant survey

Alberta Health Services is seeking feedback from Community Providers, regarding the 2011-2012 influenza immunization program. This feedback will be used to evaluate the 2011-2012 influenza immunization program, and plan for the 2012-2013 season. 

The survey is open until April 16, 2012.
