This event takes just before the Canadian Patient Safety Institute’s National Forum on Patient Safety and Quality Improvement on October 29 and 30, and is co-hosted by Alberta Health Services, the Health Quality Council of Alberta, and the Canadian Patient Safety Institute.
The pre-conference day will feature local, national and international presenters on topics related to identifying and addressing gaps in transitional care. Transitions in care refer to points in the patient journey where an individual may see different providers, different programs, and where communication and care planning is of utmost importance.
Sessions and speakers are outlined in the pre-conference program.
