New guideline: Prevention and Management of Cardiovascular Disease Risk in Primary Care

Toward Optimized Practice (TOP) has released a new guideline, Prevention and Management of Cardiovascular Disease Risk in Primary Care. The guideline provides measures to streamline and simplify the management of cardiovascular disease risk, and provides suggestions on how clinicians can identify patients most likely to benefit, while actively involving patients in their own care.
