Regulated members may wonder what happens to the information about practice incidents and close calls documented and submitted through their pharmacy’s practice incident management platform. The information is collected anonymously by the Institute for Safe Medication Practices (ISMP) Canada’s National Incident Data Repository (NIDR). ISMP Canada analyzes the data and turns it into practical advice that pharmacy teams can use to prevent future practice incidents and close calls.
The learnings from practice incidents and close calls across Canada are presented in a variety of formats, including ISMP Safety Bulletins, NIDR Safety Briefs, and the NIDR National Snapshot.
A fourth tool is the quarterly SMART (Specific, Measurable, Attainable, Relevant, and Time-based) Medication Safety Agenda, which turns learnings from incidents reported to the NIDR into practical guidance that community pharmacy teams can act on. This new resource pairs anonymized incident examples with recommendations mapped to the Hierarchy of Effectiveness and a ready-to-use SMART action plan template to support team discussion, documentation, and continuous quality improvement.
