The Institute for Safe Medication Practices (ISMP) Canada has engaged an independent research and evaluation firm to evaluate ISMP Canada’s activities under the Canadian Medication Incident Reporting and Prevention System (CMIRPS) between April 1, 2023, and March 31, 2026.
This survey asks about your awareness and use of ISMP Canada’s medication-safety products and activities, how they have informed your practice, and their influence on medication-safety practices and improvements. The results will help assess the reach, usefulness, and contribution of ISMP Canada’s work and identify opportunities for future improvement.
ISMP Canada resources can support pharmacy teams in reviewing incidents and close calls, identifying opportunities for improvement, and considering changes to pharmacy processes. The information for these resources comes from the practice incident and close call reporting being submitted by pharmacy teams through CQI+. The survey also provides an opportunity for regulated members to reflect on how they use these resources in their practice, including the challenges encountered and factors considered when implementing practice changes.
Participation is voluntary. The survey should take approximately 10 to 15 minutes to complete. You may skip any question that you do not wish to answer. No individual respondent will be identified in evaluation reports. Any open-ended comments used in reporting will be presented without identifying information.
Anyone is eligible to participate in the survey if they use, implement, disseminate, endorse, regulate, or are otherwise affected by ISMP Canada’s CMIRPS-funded products and activities. This includes ISMP Canada publications used by both community and hospital pharmacy professionals.
Responses will be accepted until October 2, 2026.
