As pharmacy practice continues to evolve, ACP has revised the Chat, Check, Chart (CCC) tools to better support pharmacists in meeting standards. The CCC tools have been a key resource for over a decade, supporting pharmacy teams in delivering patient care. They have now been redesigned to better align with current standards, and the University of Alberta Faculty of Pharmacy and Pharmaceutical Sciences’ Patient Care Process.
This article is the first in a series that will appear in The Link over the next few months to introduce the latest evolution of the CCC tools, available now on the ACP website. The tools are designed to be flexible and adaptable, allowing pharmacy teams to use them in ways that fit their workflow, practice setting, and patient needs. The tools support clinical decision making and help translate standards into everyday practice, supporting person-centred care. Pharmacy teams are encouraged to review these tools and reflect on how they can integrate them into their own practice.
